Amy Jo (Fatherly) Velez 50, of Salem, passed away on Monday, September 30, 2024, peacefully at home with family, following lengthy health complications.

She was born December 18, 1973, in Salem, Ohio, daughter of the late Paul C. and Patricia L. (Spencer) Fatherly.

Amy lived in many places in Ohio and Florida throughout her life but most recently in the Lisbon/Salem area. Amy was well known for being outspoken and not shy in any way. She was very passionate when she set her sights on a goal. Amy loved her family with all of her heart and that love was returned to her by them. She will be so dearly missed by all who knew her.

She is survived by her husband, Shantol Velez, whom she married July 22, 2006, daughters Dallas Helman and Robyn Parrish, son Sebastian Velez, step-son James Bannerman, her siblings; Steve (Amy) Fatherly, Susan (Victor) Moser, Rhonda (Jeff) Coil, and Bruce (Nicole) Fatherly, grandchildren; Gabby, Emmet, Riley, and Kameron, as well as numerous nieces and nephews.

A Celebration of life gathering will be on October 20, 2024, 1 PM, at Firestone Park, Columbiana, Ohio, pavilion #1.
